The Minister of Health, Olivier Véran, decided among the recommendations presented this Tuesday, July 21 by Nicole Notat, at the end of 50 days of animation of the debates of the Segur of Health. He retained 33 of them. Many good intentions, the will to go “faster, further, stronger”, but few details presented.
We have already spoken of wage measures, validated by an agreement with the unions on July 13, representing an effort of 8.2 billion euros per year. On the investment side, the state takes over a third of the debt (13 billion euros) of hospitals and an envelope of 6 billion euros will notably allow the renovation of a quarter of places in retirement homes. Among all these measures, here are seven that should change the lives of patients, beyond improving the quality of care.
More beds. Within the AP-HP (Assistance publique-Hôpitaux de Paris), 12,000 beds have been closed in 28 years. And every winter, an epidemic revives the controversy over the lack of places. Olivier Véran has therefore announced a moratorium on deletions and undertakes to "open or reopen", from winter 2020-2021, 4,000 beds "on demand", financed from an annual budget of 50 million euros. No more stretchers in the hallways? Will some 4,000 beds be enough?
Hotels for patients. Olivier Véran wants to generalize the experimentation of hospital hotels from 2021. These are non-medical structures allowing patients to settle on time, and at low cost, near the hospital where they must receive care during the day. Hospitals can offer an internal offer or enter into an agreement with an external structure. What to simplify the life of the patients far from the hospital.
Direct access to the hospital for the elderly. When a resident of nursing home (accommodation establishment for dependent elderly people) falls ill or is injured, it is the promise of complications and congestion in the emergency department. Ségur drew lessons from the health crisis by renewing a system that has proven its worth: direct access to the geriatric ward. A course that will have to be organized and set in stone. As for the law on dependency, Olivier Véran promises it for "end of spring, beginning of summer 2021".
Teleconsultations always reimbursed at 100%. Faced with the success of the teleconsultation during confinement, and the absence of drifts, the minister announced the maintenance of their 100% support. The obligation for the doctor to know the patient will even be relaxed. The minister also wants to extend reimbursed teleconsultation to specialists, midwives and dentists. The complementary health will have to put their hand in their pocket.

An SAS in each department. A digital platform, the future Access to Care Service (SAS) will have to respond to all requests, by phone or Internet: search for information, an appointment with a city doctor, reporting a serious emergency. Created in partnership by Samu and city doctors, these SAS will open in each department from the end of 2020.
A commitment to disability. Because two-thirds of people with disabilities do not have an attending physician, their recourse to gynecological care is 17 points lower than the national average… Olivier Véran wants a “support commitment” mobilizing experts and creating a dedicated course.

Reinforced psychological support. The confinement imposed to fight against Covid-19 has shown the need for psychiatric or psychological support within the population. The minister therefore wants to create a national suicide prevention number and an offer of psychological care in multidisciplinary health homes.
