The World Health Organization over the weekend approved for emergency use the Chinese-owned Sinopharm vaccine, further opening the door to expanding its global vaccination campaign.



While here in Israel the unprecedented vaccination campaign has led to an immunization rate of about 60 percent, the corona wards have been emptied and a return to routine is already well felt, in many countries around the world the situation is completely different.

For example, in the African continent only one percent of the population is resilient to corona so far, and in India alone about 360,000 new corona patients have been added and over 3,500 patients have died every day in the past week.

The global shortage of corona virus vaccines has led to a situation where high-income countries are over-vaccinated, while middle- and low-income countries are being pushed to the end of the queue against vaccine manufacturers.

Logistical difficulties in transporting and delivering vaccines also make accessibility in developing countries difficult.

For example, the vaccines used in Israel must be stored and moved at temperatures below zero: minus 70 degrees for the Pfizer vaccine and minus 20 degrees for the Modern vaccine.

So beyond the initial cost of each vaccine dose, the complex logistics keep the hands of residents of developing countries out of reasonable access to those expensive “cold” vaccines.



In contrast, the vaccine from the Chinese company Sinopharm belongs to the group of "hot vaccines", which can be stored and transported at the temperature of a regular refrigerator.

Similar to the flu vaccine, Sinopharm's corona vaccine is based on inactivated viruses, which encourage the body to produce antibodies to neutralize and eliminate the corona virus in case of infection.

All clinical trials to test the effectiveness of the vaccine in preventing coronary heart disease have been conducted outside China due to the zero incidence of the disease in the country since the end of the first wave in mid-March 2020. Also, in a controversial move, in July 2020 Chinese health authorities approved the vaccine for emergency use. Exists at the time for its efficiency and safety.



According to data collected by researchers at Duke University in the United States, more than 100,000 SinoPharm vaccine doses have already been purchased by countries such as Morocco (40,000), Egypt (20,000) and Mexico (12,000). Second, the approval allows for the inclusion of the SinoPharm vaccine initiated by Covax, led by the World Health Organization, which aims to equitably distribute two billion corona vaccines by the end of 2021 to ensure their access to at-risk populations and low-income medical teams.

As part of this initiative, middle- and high-income countries participate in the financing and supply of vaccines to low-income countries, mainly in the African continent and South Asia.

Immunization diplomacy

Another reason for the widespread distribution of the Chinese vaccine around the world is that China is expected to deepen its use for "vaccine diplomacy". The expression means the use of vaccines for the benefit of achievements in the international political arena. As of March 2020, the Chinese government has used its industrial production power and transportation in favor of shipments of essential medical equipment (masks, protective equipment and respirators) to medical teams around the world, an approach dubbed "mask diplomacy". Among other things, the Chinese government has promised to provide free vaccines to countries in Africa and Southeast Asia. But as the saying goes “there are no free meals” and it is likely that these countries will have to repay China and meet its political and economic demands in due course.



Immunization diplomacy and mask diplomacy are in fact part of China's strategic complex to increase its global influence.

Since 2013, the Chinese government has been promoting an ambitious global infrastructure program called the Belt and Road Initiative.

The plan includes the development of international trade routes and infrastructure on land and at sea, along the historic "Silk Road" and beyond, while strengthening China's economic ties with Asian, African, European and South American countries.

Out of the economic and health crisis that many countries are experiencing due to the corona epidemic, the Chinese government wants to deepen its global impact and offers access to preferred conditions for corona vaccines to countries that have trade relations with it as part of the Belt and Road Initiative.

78 percent efficiency, at best.

Due to the Achilles heel of China's glorious program it is in the minority of reliable information on the effectiveness of vaccines. According to a document presented to committee members at the World Health Organization, SinoPharm's vaccine efficacy stands at about 78 percent, a low number compared to Pfizer's vaccine efficacy of about 95 percent. But while Pfizer's vaccine efficacy has been proven - both in controlled clinical trials and in real-world conditions - and has been published in the scientific press, much is hidden about the visible evidence for SinoPharm's vaccine and may actually be less effective. Just a few months ago, another Chinese vaccine called Sinovac was declared effective at 78 percent but was eventually found to have an efficacy rate of only 50.4 percent, according to a report by researchers who tested the vaccine in Brazil.



The emergency approval given by the World Health Organization for the vaccine of Sinopharm indicates that the advantages of the Chinese vaccine - cost, durability and accessibility - outweighed the disadvantages, with a serious global shortage of vaccines in the background.

It is likely that the Chinese vaccine will not be the only solution to eradicate the corona epidemic in developing countries.

However, its logistical characteristics and the political context in which it is distributed make it an important part of the puzzle, as the global epidemic will not end until Corona vaccines are accessible to all.
