Corona third vaccine: This is what the US thinks

Concurrent with the Ministry of Health's interim decision to vaccinate adults in a third dose, Pfizer has announced that a third dose of vaccine will help against the spread of the Delta variant in the world.

Still, the US is in no hurry to vaccinate. Why actually?

Last night something happened in Israel, when it will probably become the first country to vaccinate its elderly population with a third vaccine for corona.

Members of the epidemiological treatment team at the Ministry of Health have recommended the administration of a third vaccine against the corona virus.

The committee members did not agree on the age from which it would be recommended to get vaccinated (65 or older or 70) and the issue will be decided today by Prime Minister Naftali Bennett and Health Minister Nitzan Horowitz along with senior health officials.

At the same time, a document was published last night by Pfizer showing that the third vaccine, the booster, can "strengthen" the protection against the Delta variant - beyond the protection provided by the two standard doses.

The published data indicate that the levels of antibodies targeted in the delta version are five times higher among people aged 18 to 55 who received a third dose of the vaccine.

Among people aged 65 to 85, Pfizer data indicate that the levels of antibodies that need to protect against Delta are 11 times greater than after a second dose.

The data, which included tests of 23 people, have not yet been reviewed or published by colleagues.

It is not clear whether the increased antibody levels are indeed related to better protection, or whether additional protection is needed at all.

The American Centers for Disease Control and Prevention (CDC) says that current vaccines protect people well from all common versions of the virus and they do not recommend a third vaccine.

Still, Dr. Michael Dolsten, a world leader in research, development and medicine for Pfizer, called the new data on a third dose of the vaccine "encouraging." According to him, the data also show that the levels of antibodies are much higher against the original version of the virus and the beta version, which was first identified in South Africa, after a third dose.

Effective or not?

At the same time, Pfizer and its partner BioNtech have released new safety and efficacy data for their vaccine, saying the protection appears to last at least six months, although it may start to decline slightly towards the end of that period.

Data released yesterday on the online server medrxiv.org, updated the results of Pfizer's experiment which included 44,000 volunteers worldwide. The experiment found that the overall effectiveness of the vaccine was about 91 percent during the first six months after receiving it. The effectiveness of corona's severe vaccines was about 97 percent, the data show. The article has not yet been peer-reviewed and has not been published in a recognized scientific journal.

The data show that vaccine effectiveness has peaked at more than 96 percent a week to around two months after receiving a second dose of vaccine, then appears to have gradually dropped to 83.7 percent four to six months later, with an average decline of about 6 percent in the past two months.

It is not yet clear who needs another vaccine.

Despite these findings, Israel is said to be the first country to probably approve a third vaccine, the FDA and CDC said in early July that Americans still do not need third doses and that the decision when another dose will be needed does not depend solely on drug companies.

The problematic nature of a third vaccine stems from both health and moral desires.

Experts say it is still unclear how antibody levels correlate with real-world immunity, and to what extent other parts of the immune system - such as B cells and T cells - can cause protection.

In addition, the question of whether it is moral to recommend a third booster injection in some countries while there is a large shortage of vaccines in the developing world is a "critical question" because the ability to reduce the chance of further variants largely depends on the spread of the virus worldwide.
