A case of an American man who lost vision in his right eye for two days could provide further insight into how the corona virus disrupts the immune system in ways that cause the body to attack itself.



The 70-year-old patient, whose name was not released, suffered from a runny nose and a corona test found positive for the virus.

Three weeks later he arrived at the hospital with many more symptoms.

In a report released by a team of doctors from Columbia University, it was reported that the man had recovered from all the symptoms, except for one headache that came a week later.

Shortly afterwards, his vision in his right eye deteriorated, until he was able to notice only wide movements.

Every movement made in this eye also caused intense pain.

Cases of temporary vision loss due to SARS-CoV-2 infection are uncommon, but also not uncommon, with several cases suggesting possible connections.

The place where this case stands out is in the combination of symptoms, including severe headaches and discomfort in the eye itself.



A CT scan quickly revealed the source of the patient's pain - very inflamed sinuses on the right side of his face.

The case was so severe that it caused the bone to erode on its own and required an emergency procedure to remove as much of the inflamed material from each of the cavities as possible.

At first the surgery seemed to work, but very quickly the pain returned and his vision did not improve.

How cunning is this virus?

Cultures have shown the presence of a bacterium called Streptococcus constellatus which is considered to be part of the natural flora and is usually found in the oral cavity and may in bad cases cause abscesses and heart infections.

Further investigation has revealed the prevalence of an active type of white blood cell, one that usually produces a small and strange antibody called IgG4 at a rapid rate.

Unlike other antibodies, this antibody is able to reassemble itself intelligently.



It also has a reputation as a cause of its own problems through IgG4-related disease (RD) - an autoimmune condition that can affect almost any part of the body, which incorporates various diseases and can mimic diffuse vasculitis, infectious diseases or tumors.

With a diagnosis of IgG4-related rhinitis, the patient was given steroids and soon returned to good health and vision.

Three weeks later he fully recovered.

We already know that SARS-CoV-2 is a cunning and tough virus when it comes to messing with our immune response.

However, even given an earlier medical study suggesting the same potential link between IgG4-RD and COVID-19, it is too early to draw firm conclusions about a link between the two diseases.
