Four prisoners fled a closed ward of the Clinic am Weissenhof in Weinsberg (Heilbronn district) on Thursday night.
There they had apparently been in the penal system.
The four men are considered dangerous and may be armed, the police said.
The authorities warn against hitchhiking people.
Witnesses are asked to report suspicious persons to the police via the emergency number 110.
