The couple who were killed in Bavaria on Sunday had apparently been surprised in their sleep.
"We assume that they were attacked in their sleep," said a police spokesman.
Local media had previously reported on it.
The suspect 18-year-old was arrested for alleged murder in two cases.
"We're assuming a single suspect," said the spokesman.
The young man is said to have killed the 47-year-old woman and her 51-year-old husband on Sunday night in their single-family home in Mistelbach near Bayreuth.
According to investigators, the alleged perpetrator was the boyfriend of the couple's eldest daughter and therefore spent the night in the house.
According to the investigators, the four underage children of the doctor and the pediatrician were at home during the act.
18-year-old turned himself in a few hours after the fact
