Quite a few people - mostly men - suffer from an inguinal hernia, also known as kila.

Dr. Eran Nazri, director of the Peritoneal and Melanoma Unit in the Surgical Division at Ichilov Hospital, was a guest on the podcast "Specialist Clinic" to explain what causes a hernia, who is at increased risk of suffering from it - and how to deal with it?

"A hernia is a condition in which certain organs from the body come out through an opening we have in the abdominal wall, or naturally or an opening created by weight lifting, in areas of weakness that we have in the abdominal wall," explained Dr. Nazri, adding that a hernia is more common in men because they have An opening through which the seminal duct enters the testicle, "and this opening tends to expand."

In fact, a hernia is a condition in which an organ or fat from the abdomen comes out through the same opening and causes pain.

"When the hernia widens a lot, intestines can also come out through this opening," Dr. Nazri explained. Which supply the intestine, then the intestine can undergo necrosis.

"

And yet, it is important to emphasize that in most cases it comes out of the hernia, into the scrotum or bulges in the groin, only fat, and then the person will feel pain in the groin. Not necessarily seen an external hernia, and usually It is found when people complain of groin pain during exercise.

Who is at risk of rupture?

"The classic patient is a young and physically active person, usually a man, although there is an influenza hernia in women as well but it is less common. Not necessarily weightlifters, but also runners or people who are active in the gym," Dr. Nazri explained. Dimly dull during exercise, and it stops after the activity.Because at first the pain is dull, there are quite a few people who turn to treatment only when the hernia is larger, and more painful.

Common in young men who are physically active.

Aside from exercise, anything that raises intra-abdominal pressure can increase the risk of rupture, such as heavy lifting, chronic cough, constipation, men who have difficulty urinating in an enlarged prostate wave, and more.

And there are also people who are just born with a hernia.

How is it diagnosed?

A family doctor will refer to the surgeon based on the complaints, and the surgeon will do a physical examination in which he will feel the hernia.

"You can make a patient cough while inserting a finger into the groin canal, and then you can feel the hernia. This is a very accurate test, even better than an ultrasound test," Dr. Nizri explained.

How is it treated?

First it is important to clarify that a hernia is treated only when it causes a disruption to the patient.

"Asymptomatic inguinal hernia, which does not bother patients at all, it is perfectly legitimate to offer follow-up of physical examinations, to see what happens with it," Dr. Nazri explained, "and we know from follow-ups done by many people that in 10 years, 70 percent of people become Symptomatic.

In this case, what I always recommend to patients is that they do not always have to be repaired just because there is a hernia and someone saw it by chance on ultrasound, but also do not wait for a situation where there is a big ballet like a ping pong ball in the groin, because it is difficult and unpleasant. A lot of time".

The hernia repair surgery is performed in two approaches: surgery with an open approach, with a small incision in the groin, and the hole through which the organs come out is closed using a mesh and then the groin is closed.

The second approach, which is more common today, is laparoscopic, "a minimally invasive approach, accessible precisely from the navel to this hernia, the principle is the same principle - close the hole using a mesh - but the approach is in smaller incisions and is made through the abdominal wall."

In this method the recovery is faster, and it allows repair of both sides with the same incisions.

Usually hospitalized for the night after surgery.

The surgery is not complicated, but placing the net in the groin area can cause pain for two weeks to a month.

"Discomfort as a result of a foreign body in the groin," explained Dr. Nazri. In 5 percent of cases, patients develop chronic pain, but even that has medical solutions today.
