According to police, the shooter at a hospital in Tulsa, Oklahoma, killed his doctor, apparently because of back pain after an operation.
Tulsa Police Chief Wendell Franklin said the attacker killed his doctor, a female doctor, a receptionist and a patient on Wednesday and then shot himself.
The man bought an automatic rifle on the day of the crime and shortly before that a handgun.
Police Chief Franklin also said a letter was found on the man that made it clear that he wanted to kill his doctor "and everyone in his path."
The man blamed the doctor for ongoing problems after a reverse operation on May 19.
The United States is currently being shaken by a series of shootings.
On Tuesday last week, an 18-year-old killed 19 children and two teachers at an elementary school in Uvalde, Texas, before being shot by the police.
A few days earlier, a man had opened fire in a supermarket in Buffalo, New York, killing ten people and injuring three others.
According to investigators, the crime was racially motivated.
He was charged with terrorism and ten counts of murder.
