78-year-old Nachshon Har-Zvi

is the man who died yesterday (Wednesday) at Beilinson Hospital in Petah Tikva during a complex heart catheterization operation, this after the x-ray system, which allows the reflection of the blood vessels for the catheters, stopped working.

"Israel Hayom" learned that a few weeks ago there was another malfunction in the X-ray system and a repair was ordered for the device.

During a routine catheterization operation at Billinson Hospital, the X-ray system stopped working.

Har-Zvi was left with the equipment used for catheterization inside his body, when the catheterists could not continue the catheterization without seeing what was happening inside the body, and on top of that they could not pull back the equipment.

In coordination with the Ministry of Health, it was decided to establish an inspection committee at the hospital.

At the same time, representatives of Philips, the manufacturer of the device, are on their way to Israel to check the malfunction.

In addition, the catheterization treatments continue in the hospital as usual in another room where the systems work as they should.
