While statistics relating to the number of confirmed cases of Covid-19 and that of hospitalized patients have been rising in our department since the beginning of November, the Groupe hospitalier sud Ile-d-France (GHSIF), located in Melun, is already seeing it. within its walls.

Friday, November 6, there were 129 people hospitalized due to the coronavirus, including 18 in intensive care.

An exponential increase compared to October 15, when it hosted 29 Covid hospital patients including 4 in intensive care.

That's four and a half times more in three weeks.

The positivity rate in the hospital screening center was 21.4%.

“The second wave we're talking about, here we are.

And the increase in the number of Covid-19 cases is faster than in the spring.

At the height of the crisis, we had 220 patients hospitalized for Covid, ”recalls Dominique Peljak, director of the GHSIF.

Melun, October 2018. “The increase in the number of Covid-19 cases is faster than in the spring”, confirms Dominique Peljak, director of the GHSIF.

Hence an organization that is growing in strength to welcome the sick.

By October 26, the number of resuscitation beds had increased from 22 to 34 by converting beds in the Continuous Monitoring Unit (CCU).

From this Monday, November 9, it will climb to 46.

What about classic, so-called conventional beds?

About fifteen were created after conversion of surgical beds, following the deprogramming of certain interventions.

"It is progressive and mainly in obesity surgery for the moment", specifies the director of the GHSIF.

The hospital pays 40,000 euros per month to the clinic

In addition, it pushes the walls to welcome the patients of the coronavirus.

Since November 5, 15 day hospital beds have been transferred to the private clinic Saint-Jean l'Ermitage (CSJE) which adjoins the GHSIF.

As of Monday, November 9, 15 other surgical beds will be transferred there.

This cooperation with the CSJE bristles the unions because it costs the hospital monthly rent.

"It's 40,000 euros per month to rent a wing for 30 beds," says Dominique Peljak.

"Shameful!

», Notably protested Stéphane Parouty and Valérie Christian, from the Sud Santé Social union.

"Last spring, this rental cost the hospital more than 50,000 euros while the clinic received 350,000 euros of public money in parallel," they point out.

At the time, the CEO of the clinic mobilized during the crisis had justified it by the credit which was still running on its own premises made available to the public service, as well as by the fall of 80% of its activities, the scheduled interventions being canceled as of March 12 as part of level 2 of the White Plan.

Melun, April 2020. The GHSIF will welcome 57 students from the Nursing Training Institute (Ifsi) of Melun as part of their internship to be carried out in the 2nd and especially the 3rd year.

If necessary, Dominique Peljak reserves other room for maneuver: the conversion of six other continuous surveillance beds into intensive care beds, the use of a second accommodation wing offered on the 3rd floor of the clinic.

It is not known if there will be a flat rate for the two simultaneous rentals.

If necessary ... The screening center installed in the fall in the old hospital (8 rue de Vaux in Melun) is a good barometer for monitoring the evolution of Covid-19 with a result within 24 hours.

Since May 13, this center has carried out 32,493 PCR tests, at a rate of 400 per day since the use of a second machine at the GHSIF laboratory in October.

"We are the only hospital in the department to offer this service", points out the director.

Retirees in renfont are welcome

If beds are essential to face this second wave of the coronavirus, caregivers are obviously essential.

According to the director of care, Franck Langlois, “The problem arises on the nursing side, among nurses.

We are therefore going to welcome 57 students from the Nursing Training Institute (Ifsi) of Melun as part of their internship to be carried out in the 2nd and especially the 3rd years ”.

In addition, two executives of the GHSIF carefully read the spontaneous candidacies, seek solutions from the liberals who would like to come in reinforcement.

“We continue to mobilize retirees and anyone with medical skills.

Let them be reassured: we are accompanying them.

We duplicate them with professionals ”, specifies Franck Langlois.

Exhausted by the overactivity of spring and having difficulty recovering from it, the staff were able to take their leave during the last All Saints holidays when many establishments had canceled them.

“In November, we do not touch the holidays as long as we can.

We preserve the rest of our teams, ”insists Dominique Peljak.

Melun, May 2020. The medical consultation unit made available to caregivers revealed 35 positive cases in September and 112 in October (out of 2,400 agents).

What about their health?

The medical consultation unit made available to caregivers revealed 35 positive cases in September and 112 in October (out of 2,400 agents at the Melun site).

"Since September, 23 cases have required sick leave in Melun and 6 in Brie-Comte-Robert (Editor's note: hospital which merged with that of Melun in 2017)", says the HRD.

Will of benevolence

What to do for nursing homes where absenteeism is important?

According to Valérie Christian (South), “it's a hassle, the temporary workers do not answer when they are called.

This risks causing a burn-out among the agents present who accumulate the hours ”.

Response from the HRD: “Recruitment has always been very difficult in nursing homes.

There is the freedom of individual contractors to accept or refuse.

In the event of severe absenteeism, we have a substitute service that can intervene.

We offer paid overtime.

We modify the schedules.

"

According to the South union, staff fatigue would lead some to mood swings that generate “disproportionate” sanctions, disciplinary boards.

Dominique Peljak refutes this: “We had 6 disciplinary councils in 2018, 5 in 2019 and 3 in 2020 out of 2,400 agents in Melun.

For me, there is a red line that should not be crossed: when there is malice, mistreatment or fault in the care.

But currently, we have a desire for benevolence towards the staff, I am not unaware of their physical and psychological fatigue.

We reactivated the psychological support unit.

With my team, we are here to support everyone! ”
