The Landesbetrieb Straßen.NRW admitted this on Wednesday.

The streets of North Rhine-Westphalia announced
that the “improvised construction was listed as a defect” in the
2008 acceptance
report for the structure.
It was "only accepted subject to
static evidence
".
This was not done despite repeated requests.
The
construction company is now insolvent
.

Concrete slab tears out of bracket - driver (66) was killed
Around two weeks ago, a woman was killed in her car on a heavy concrete slab on the busy Kölner Ring.
The plate fell out of the soundproofing wall onto the Cologne woman's car (66).
During the investigation it was found that the five-ton
plate had been attached incorrectly
.
After the
fatal concrete slab accident on the A3 near Cologne
, six more slabs were dismantled.
The investigation had shown that the noise protection panels were also incorrectly attached in 2008.
The plates, which had turned out to be too large, were attached with an improvised construction with a welded-in screw.
