The eighty-year-old woman was admitted to Queen Elizabeth Hospital for lung infection and heart failure, but was found to have fallen off the ventilator tube during her stay in the hospital. She died after rescue. The cause of death started an inquiry earlier.

The coroner believes that the old woman removed her throat by herself at the time, and she suffered from chronic alveolar injury, pneumonia, and congestive heart failure, and the termination of mechanical infusion caused her death.

Evidence showed that anyone would feel uncomfortable and want to pull out their throat. There was no other evidence that she wanted to commit suicide, and she died of misfortune.

In view of the fact that the situation at the time involved the professional judgment of the frontline medical staff, there was no evidence that the medical and nursing training was insufficient, and the magistrate did not make any recommendations.

The deceased Yuan Yanfen (80 years old) was found in Queen Elizabeth Hospital on January 29, 2019, when the hose connected to the ventilator was found to have fallen off and died after rescue.

Evidence in court showed that Yuan himself suffered from diabetes and high blood pressure. When he was admitted to the hospital on December 24, 2018, he was found to have lung infections and congestive heart failure. Later, he developed pneumonia and needed intubation to help breathing. The doctor prescribed a sedative at the time. In order to reduce the discomfort of her intubation, she also tied a hand strap to prevent her from touching the equipment, but the doctor later found that her blood pressure was low, so she stopped the sedative.

On the morning of January 29, 2019, when the doctor visited the room, he thought his condition had improved. However, at about 1 p.m., the nurse heard the alarm sounded in his ward. When he arrived in the ward, he saw that the hose connected to the ventilator fell off, and the hose was in place. On Yuan's palm, she died after being rescued.

The witness confessed the course of the case.

(See the picture below for details)

Think Yuan removed the pipe by himself

The death magistrate stated that Yuan suffered from chronic alveolar injury, pneumonia, and congestive heart failure at the time, and the termination of mechanical infusion caused her death.

He referred to the nurse's testimony. When the nurse arrived in the room, he saw that Yuan's body slid down, his head tilted to the left, and the throat was on her palm. He believed that she had removed the throat on her own to rule out the possibility of homicide.

However, the evidence also shows that anyone who has a throat intubation feels uncomfortable and wants to pull the throat. There is no other evidence that Yuan wants to commit suicide, especially when his condition improves, so the possibility of suicide is also ruled out.

The magistrate continued that although Yuan was in poor health, her condition improved that day, and the court had ruled that her fatal injury was the termination of mechanical ventilation, and also ruled out death from nature, and finally ruled that she died of misfortune.

There is no evidence of insufficient medical training

Regarding whether to give advice on the incident, the magistrate believed that whether to use sedatives and whether to adjust the length of the restraint hand strap belonged to the professional judgment of frontline doctors. There was no evidence in the case that the medical training was insufficient. Suggest.

Regarding whether a dedicated person is required to monitor the closed-circuit television of the ward, the magistrate believes that there is no delay in rescue because there is no dedicated person to monitor the closed-circuit television of the ward. Moreover, through the closed-circuit television, it may not be possible to see the situation of the throat. The magistrate believed that the situation was different, and made no suggestions on these two points.

The magistrate finally told the family that he understood that they felt painful and helpless in the incident, but hoped that the hearing would relieve them of their doubts and get out of the gloom as soon as possible.
