Cancer is the number one killer disease in Hong Kong. Many people have experienced chemotherapy, radiotherapy, and targeted drugs that are ineffective, and they have switched to trying "immune cell therapy."

The price of this treatment is high, with each needle exceeding 100,000 yuan. Some people will fly to Japan for treatment, and some will crowdfund medical expenses. Therefore, this term has gradually become commonplace in the public eye.

Ou Wing-jen, a specialist in hematology and hematology oncology, pointed out that the only legal immunotherapy in Hong Kong is "CAR-T (Chimeric Antigen Receptor) Cell Therapy". Sapai Port’s Liu Zihao defeated blood cancer was received at the University of Hong Kong and Queen Mary Hospital. It is also the first clinical treatment outside the trial. The School of Medicine of CUHK also has CAR-T clinical research.

"Immune cell therapy" is a kind of advanced therapy, mainly used to treat cancer, extract one's own immune cells, enhance their effectiveness, and then return them to the body.

Ou Yongren, a specialist in hematology and hematology oncology, pointed out that the legally available cell therapy in Hong Kong is currently only CAR-T cell therapy, and there is no NK cell or CIK cell therapy.

Hong Kong University and CUHK can do CAR-T therapy

"CAR" in CAR-T therapy is the abbreviation of "Chimeric Antigen Receptor", that is, "chimeric antigen receptor", and "T" refers to "T cell", a type of lymphocyte, which plays a role in immune response. important role.

CAR-T therapy is about to extract "immune T cells" from the patient's blood for genetic modification, so that the T cells can identify cancer cells and return them to the patient's body to achieve the effect of eliminating cancer cells.

Ou Wing-jen said that the treatment is registered by a pharmaceutical factory and currently only targets two types of cancer: "Currently, Hong Kong can only be licensed in certain hospitals and executed by a few teams."

Currently, Hong Kong University has clinical treatments other than CAR-T trials, while CUHK has clinical trials.

Sapai Port will receive CAR-T therapy for Liu Zihao to defeat blood cancer

Sa Pai Port's victory over blood cancer by Liu Zihao was the first time that he received CAR-T therapy at the University of Hong Kong and Queen Mary Hospital in October last year. It was also the first time in Hong Kong to use this therapy to treat blood cancer patients outside of a clinical trial.

The drug Liu Zihao accepted is "Tisagenlecleucel", which is the first and only CAR-T cell therapy approved in Hong Kong. It was registered with the Department of Health in March 2020 under the Pharmacy and Poisons Ordinance.

According to the Hong Kong Index, Tisagenlecleucel involves a complex process, including: 1. Collecting patient’s lymphocytes; 2. Cryopreserving the cells with liquid nitrogen; 3. Transporting the cells overseas for genetic modification; 4. Transporting the cells back to Hong Kong after modification.

The Faculty of Medicine of CUHK also launched CAR-T clinical research in 2019, providing treatment for patients with blood cancer in the first phase.

In August 2019, the Chinese University of Hong Kong announced that it will carry out clinical research on cell therapy using chimeric antigen receptor T-cell (CAR-T cell) technology. The first phase will provide treatment for patients with blood cancer. Improve patient survival rate and prolong survival.

NK and CIK therapy are still in the research stage

As for "NK cells" and "CIK cells", they refer to "Natural Killer" cells and "Cytokine-induced killer" cells respectively. They are also different immune cells in the human body.

Ou Yongren said that the current international NK and CIK cell therapies are still in the research stage, and there is no therapeutic course that can be sold in the market: "As far as I know, even in the Mainland, there are currently no cell products that can be sold. Therefore, it should only be of a research nature.” He said that some Hong Kong people would go to the Mainland, Germany, or Japan, and other countries where cell therapy is more popular, for treatment.

Qian Guowei and Zheng Weitao went to Japan for treatment

Qian Guowei, the former gold medal producer of TVB variety show, died of illness in October last year. He went to Japan to receive NK immune cell therapy during his lifetime.

And Zheng Weitao, one of the founders of "Itazen Sushi", also revealed in court that he had gone to Japan to undergo cell therapy to treat pancreatic cancer.

Looking at the information, there are companies in Hong Kong that provide patients with services for NK cell therapy in Japan.

In 2012, the DR medical cosmetology accident that caused one death, one amputation and one disability was CIK therapy. The case refers to the fact that the doctor had gone to Guangzhou to learn the therapy.

Risk of infection and rejection

Cell therapy is dangerous, and Ou Yongren emphasized: "This is always a blood transfusion procedure, and there are certain risks, such as bacteria in the blood used, and rejection of the patient." He cited, for example, that if the cells cause a patient's rejection, it can lead to Bone marrow failure; if T cell therapy is not used for matching, the injected cells may attack the patient's body and cause organ failure.

Currently foreign countries are studying CAR-NK therapy, which is called "the next generation of cell therapy."
