This resident of the Zemgor nursing home in Cormeilles-en-Parisis knows that he is benefiting from a first with this experimentation in oral-dental tele-expertise… at the foot of his bed.

It has been set up since June 7 by the Géronto 95 network - to which most of the nursing homes in Val-d'Oise, the Simone-Veil hospital in Eaubonne and the Chantepie-Mancier Foundation in L'Isle-Adam belong.

The system is subsidized by the Conference of Funders of Val-d'Oise, that is to say the departmental council and the regional health agency 95.

"I don't know if there is anything you can do for me?"

I am losing my teeth and my dentures have not held up for a month or two ”, confides the octogenarian to Sébastien Grosset, coordinating nurse in telemedicine seconded by the Eaubonne hospital two afternoons a week.

Eighteen patients have already been examined in this nursing home.

Equipped with a camera in the form of a battery-powered toothbrush, he inspects his oral cavity, teeth, tongue and cheeks included, but also his dental apparatus.

The images are recorded and will be completed by an X-ray of a “disturbing” molar, again carried out on site by a medical electroradiology manipulator from the company X-Raymobil.

The film and the radio will then be transmitted to a stomatologist at Simone-Veil hospital via the Ortif secure platform. The hospital practitioner returns his diagnosis to the establishment using the same channel, within a week, against three to six months of waiting for a classic appointment. Three possibilities thereafter: the treatment can be carried out within the establishment by the caregivers, at a dentist "in town" or by a hospitalized stomatologist.

For the octogenarian, it is also an opportunity to exchange with the teleoperator in a more familiar setting.

"Look, there, I have my decoder with Canal + to be able to watch the bike on TV," he rejoices.

“Imagine the stress it is to go for a consultation, it would have taken him the morning,” notes Sébastien Grosset.

“Before, when I had to see a specialist, I was taken by ambulance,” smiles Daniel.

Better access to healthcare and less travel costs

“For patients, this saves many trips and allows better access to care.

We gain in reactivity in its orientation.

For the establishment, this makes it possible to make a link between the liberal and the hospital world.

And this represents savings for society, by avoiding travel costs and by doing prevention ”, underlines Mathieu Humbert, project manager at Géronto 95.

The original idea comes from doctor Christian Batchy, geriatrician and director of the Chantepie-Mancier Foundation.

“We noted a major problem of access to oral health care for elderly people in nursing homes, with an impact on nutrition, walking and other aspects of health.

Among the most frequent problems encountered during examinations: caries, gingivitis, tartar, infectious abscess, joint or mandibular problems, oral yeast infections, etc.

"The interest is to take the whole of the oral sphere into account, the care is much better by a stomatologist than by a dentist", underlines the doctor François Chary, coordinating doctor of the Zegmor nursing home.

About thirty residents per nursing home will benefit from it

"The idea is not to send all residents to see the teleoperator, we are targeting a population that can benefit from it," adds Dr Chary.

Residents who have difficulty moving around or show resistance to treatment are therefore selected as a priority.

About thirty nursing homes will be able to benefit from it.

Six other nursing homes in the sector will subsequently benefit from it.

Before the device may be extended to the entire department and to nursing homes for people with disabilities.

“We started with a working group on the management of undernutrition issues in nursing homes and we now come to oral-dental telemedicine.

It's a hell of a lot of progress in ten years, ”concludes Laurence Delmar, president of Géronto 95.
