Kowloon Lok Sintang has served Hong Kong for more than 141 years, and its services are very diversified, including medical care, elderly care, and social welfare.

Seeing that older people in the community need more attention on cognitive issues, with donations from the Hong Kong Jockey Club Charities Trust, Lok Sin Tong has been working with the Jockey Club Senior Citizens to promote the Jockey Club "Walk with Seniors" since 2019. "Dementia Support Program.

The plan uses the model of "early detection, early diagnosis, and early intervention" to provide appropriate services for people suspected of having mild cognitive impairment and early dementia, and at the same time enhance the confidence and ability of caregivers of people with dementia.

Facing confusion with mild cognitive impairment

Ms. Su found that her mother Xiangjie’s memory began to decline, and she often forgot something, such as occasionally forgetting to press the cooking switch of the rice cooker, skipping to buy food or repeatedly asking some questions, etc., emotionally unstable and easy to feel depressed.

Miss Su didn't know how to help her, and worried that Sister Xiang's memory would deteriorate further, so she took her mother to participate in the cognitive ability test and evaluation of the Leshantang "Walking with the Senior" project.

After evaluation, Sister Xiang confirmed that she was suffering from mild cognitive impairment, which is a "transitional period" between normal aging and early dementia, and is an early manifestation.

Family intervention "people-oriented" care model

After the evaluation, Ms. Su often encouraged her mother to do more cognitive training at home, but her mother's motivation was low and she was reluctant to do it often. Ms. Su was very troubled by this at first.

Through the caregiver support service of "Go with the Elderly", the case manager of the project patiently followed up with Ms. Su's situation through the phone on a regular basis, exchanged opinions, and discussed care measures together.

Knowing that Sister Xiang is not very interested in cognitive learning and training, the case manager suggested that Ms. Su use more creativity, such as increasing Sister Xiang's interest through the diversified tablet computer and interesting brain games.

So Miss Su deliberately brought her tablet to play some brain games for her mother to use her brains and relieve her boredom.

Sure enough, Sister Xiang was very engaged.

According to the needs of the elderly with dementia, we will continue to review and improve the ways of caring for them. Caregivers and supporters need to work together to provide patients with "people-oriented" loving care from a new perspective through immense patience and unlimited creativity.

Caregiver support services also include communication strategies, emotional processing, etc., while enhancing the experience and skills of caregivers to reduce their stress.

Diversified and interesting online brain games

Project colleagues invited Sister Xiang to join the project's "I can" self-management group to communicate with others and meet new friends.

In the group, Sister Xiang actively participated, exchanged and learned with different group members, smiled all over her face, and performed some brain-building activities, such as laughter yoga, crafting, etc.

The management team also includes stress and emotion management, short-term and long-term care plans, pre-defined medical plans, and community resource strategies, so that participants can learn to take care of themselves, establish a healthy lifestyle, and prevent brain degeneration.

At the end of the group, Sister Xiang thanked the staff for giving her a happy time. She said that after the group training, she was more confident in taking care of herself and could reduce her dependence on her daughter.

The staff was also very encouraged to hear that she could enjoy it.

Seeing Sister Xiang’s smile and progress in her studies, Miss Su also let go of a lot of worries. She expressed her gratitude to Le Shantang’s colleagues for their patience and help, and she was fortunate that she was looking for the intervention and assistance of colleagues who "walked with the elderly" as soon as possible, which made Sister Xiang The situation has improved.

Group members learn muscle strength exercises together to strengthen the body and the brain.

Self-management team members design a healthy life schedule

Participants actively do brain exercises to train their minds.

The one-stop service of "early detection, early diagnosis, and early intervention" helps elderly people in need and their families receive fast and appropriate support.

So far, Lok Sin Tong has successfully served more than 2,700 elderly people or/and their families living in Kowloon City, Wong Tai Sin, Yau Tsim Mong, Sham Shui Po and Tsuen Kwai Tsing districts.

Leshantang’s colleagues support patients and their families with love, and "walk with the elderly" together.

Le Shan Tang hopes to continue to walk with more people in need and their families, and walk a life journey full of challenges and love together.
