A female patient was examined by a computerized tomography scan at our hospital on the 16th of this month due to a lump in the kidney. Under the guidance of ultrasound, a needle biopsy was performed to take out a sample of kidney tissue for pathological examination to assist the doctor in diagnosing the condition and formulating a treatment plan.

According to medical records, the radiologist successfully removed the sample during the needle biopsy process.

After verifying the identity of the patient on the spot, the medical staff pastes the patient's information on the sample container for identification, and places the container in a sealed collection bag for arranging to be sent to the pathology laboratory for testing.

The next day, the staff of the Pathology Department found that there was no tissue sample in the sample container during a laboratory test. They immediately notified their supervisor and related departments. The hospital immediately searched for the relevant sample but could not find it.
