Gene Phillip, a 76-year-old Rehovot resident, asked to be vaccinated against the flu but was given a fourth immunity to Corona instead.
According to the findings of an examination by the Ministry of Health and the HMO of which it is a member, it appears that a human error, or lack of system update, resulted in them not noticing that it had already been vaccinated three times.
The details of the case reveal that Philip, who was recently vaccinated with the third vaccine, arrived at a community center in the city of Rehovot, at the position of Clalit Health Insurance Fund, in order to be vaccinated with the flu vaccine.
At this point, as elsewhere, there are also immune positions for corona.
After her details were absorbed she received the vaccine and returned home.
When she got home, Philip checked the vaccine certificate given to her in writing and noticed that she had been vaccinated against corona, and it was written in the certificate that she had been vaccinated for the third time to corona.
But as mentioned, she was already vaccinated.
At first she thought she might have been wrong and went to the wrong vaccine center, but even a friend of hers who came to get vaccinated told her that at that point there were vaccines for both the flu and corona.
She later reported the mistake to her doctor, who reassured her and advised her to report the case and there would be any unusual side effects.
"I feel fine. The doctor reported in the file that I received a fourth vaccine. I think it's just important to find out details of everyone who comes. Maybe it's better not to have flu and corona vaccines in the same place."
The Corona virus information headquarters stated that there is a directive for all vaccination centers to thoroughly check and verify the details of the warehouse, as well as to verify which vaccine is available.
Following this case, the procedures were refined.
