Gymnast Simone Biles, who revealed her mental health issues during the Tokyo Olympics, says she is "still afraid to do gymnastics."

In an emotional interview on NBC's "Today" show on Thursday, Biles said he's still struggling with the "twisties" while performing on his Gold Over America tour.

Twisties are a mental block in which competitors can lose track of where they are while spinning in midair.

"I don't do spins (on tour). I do double half-outs, which is my trademark move on the ground. That never got to me. But everything else weighs so much, and I watch girls do it, and it's not the same." said Biles, possibly the greatest gymnast of all time.

"But the spins, once I do them, they will come back. But I'm still scared to do gymnastics," he added.

Biles, 24, who has won seven Olympic medals, including four golds, battled mental health issues and the unexpected loss of a family member during the Summer Games.

'I am happy to be a leader for survivors'

Last month, Biles testified before the Senate Judiciary Committee on the FBI's handling of allegations of sexual abuse against former US gymnastics team doctor Larry Nassar.

Biles, along with McKayla Maroney, Maggie Nichols and Aly Raisman, all of whom were abused by him, focused on how the FBI failed in their investigation and the intimidation that came from the sport's governing bodies.

"For so many years, going through everything I've been through, putting on a facade, I'm proud of myself and I'm happy that I can be a leader for survivors and give value to all who speak, so I'm happy to be a voice for them, "Biles said on Today.

At the Olympics, Biles retired during the team final and then retired before the general and individual finals of jump, uneven bars and floor exercise.

He competed in the balance beam final, winning bronze.

"Doing something that I've done forever and not being able to do it because of everything I've been through is really crazy, because I love this sport so much. It's difficult, I'm sorry. And I don't think people understand the magnitude of what I'm going through." Biles said, his voice cracking.

Biles said she has many techniques to help with her mental well-being, including using the "Cerebral" mental health app.

She is now the director of impact for the company.

"Getting the mental health therapy I need has really been a relief to me, especially while on the road and on the road. I'm happy to have such a great app to help me with that," he said.

Biles said she also likes to walk outside and enjoy a little "retail therapy."
