Last week the agreement to shorten the shifts for medical interns was solemnly announced by Health Minister Horowitz and after a few days the interns adopted him and congratulated the Minister on his courage.
Less than 24 hours later, hospital administrators opposed the outline, which was supposed to be agreed upon by them, claiming that it would harm the Israeli medical system, interns and patients.
Today the Medical Association joined them, and its members claimed that they welcome the shortening of shifts but not in the way it was signed when it is not clear how many standards and manpower will be added to the system.
The Israel Medical Association (Hari), unlike other unions and workers' unions, not only outlines and supervises doctors' salary arrangements, but is also responsible for the interns' medical training and sets the threshold conditions for obtaining a doctor's certificate in Israel according to its syllabus. Opposition by the Histadrut may overthrow the entire move.

Until the collective agreement is signed with the IRA, no other agreement has meaning, even if it is supposedly budgeted. So, at the moment there is no signed outline and all the celebrations are free.

The gap between the outline of the RI and the prescription organization and Minister Horowitz lies in the shortening of shifts for the surgical interns. Compared to the other specialties, where there is a consensus that the shifts can be shortened, there is a difficult dispute in the health system as to whether the shifts of the surgical interns can be shortened. And if they shorten it, will it require them more years of internship compared to the others, since there are hours of study that they must go through and it is not possible to deviate from them so that they know how to analyze in the best way.
But not only the surgeons, the Hari and the Ministry of Health do not yet know how to start the outline in the periphery. This outline will start in ten hospitals in the periphery in April 2022, but no one has yet figured out its implications. If this affects their years of internship so that instead of 4 years their internship will be 6 years, the interns will walk away, when even today there is a serious staffing problem of standards.According to data from the Ministry of Finance and Health, there are 100 unmanned standards of interns in peripheral hospitals.

The fact that all these questions remained open while such an agreement had already been "signed" indicates a serious problem for the Minister of Health and his ability to unite all the organizations around him.
It may have been necessary to make the IRA move against their will as well, but if in the end in April the outline does not start because the IRA will refuse to sign - everyone will be lost.
The interns will lose because they do not get what they were promised and their protest will fade before it achieves its goal, the RAs will be perceived as inattentive and solely responsible for the collapse of the outline, and the patients will not receive the most appropriate treatment because the interns will continue to work Minister Horowitz, who will be able to appropriate the outline for the next election campaign, even if it has completely collapsed, and distance himself from responsibility for it.
