After arriving at the Accident and Emergency Department, he suffered cramps again and was classified as a critical case. After the initial treatment, the patient's condition improved slightly. He was transferred to the internal medicine ward at about 8:15 p.m. for hospitalization and required examinations.

According to the examination report, the patient's brain scan, electrocardiogram, heart enzyme index, blood pressure, and blood oxygen readings showed no abnormalities.

At about 4:30 in the morning, the staff in the ward arranged the patient to take an electrocardiogram according to the instructions. This time the electrocardiogram showed an abnormality. The doctor on duty then reviewed other clinical signs and blood test reports, but failed to make a diagnosis of myocardial infarction in a timely manner based on the abnormal electrocardiogram; Only in response to the patient's vomiting at the time, prescribe anti-emetic drugs and continue to observe the patient.

At about 11 o'clock in the morning, the patient expressed chest discomfort to the attending doctor. The doctor immediately reviewed the patient's medical records, arranged blood draws, emergency electrocardiogram examinations, and prescribed nitroglycerin.

The cardiologist participating in the consultation decided to arrange for the patient to be transferred to the Cardiology Intensive Care Unit and undergo the Tong Bo Tsai Surgery.

However, the patient's condition deteriorated rapidly, with cardiac arrest and loss of consciousness at 1 pm, and he needed immediate first aid and arranged intubation to assist breathing.

The attending doctor called the family members to inform the patient that the patient's condition had deteriorated rapidly, and asked the family members to consider whether to not perform CPR at the moment. The hospital admitted that the practice disappointed the family members.

The patient suffered cardiac arrest again at 2 pm. The doctor continued to give him cardiopulmonary resuscitation and cardiotonic injections, but the patient passed away at 2:32 pm.

Tuen Mun Hospital stated that it was very concerned about the accident. The hospital representative had met with the family of the deceased on the 3rd of this month and admitted that Tuen Mun Hospital had made an error in clinical judgment regarding the patient’s myocardial infarction, and had not fully taken care of the attending doctor’s notification of the family’s condition. The family members expressed their deep apologies.

The case has been transferred to the Coroner for follow-up and notified to the HA Head Office through the Early Incident Notification System.

Tuen Mun Hospital promised to actively follow up and propose improvement measures to avoid similar incidents, including the internal medicine department promptly reminding doctors to pay close attention to changes in the patient's ECG, and doctors and nurses to strengthen communication and provide appropriate treatment for serious cases.
