Two firefighters Zhang Yaosheng and Xu Zhijie were killed in the operation. The deadly inquest revealed that the fire was caused by a malfunction of the air conditioner. Based on the complex structure of the mini-silo, it took nearly half an hour after the fire broke out that no one called the police, which led to the rapid escalation of the fire; the firefighters present were equipped with outdated walkie-talkies, Zhang and Xu were unable to activate the emergency rescue procedure commonly known as "Mayday" when they lost contact with the fire.

Coroner Huang Weiquan ruled today (11th) that he died unfortunately. Huang Guan praised Zhang and Xu for their bravery and did not flinch when they invited Ying into the fire. Their behavior was admirable.

Zhang was rescued 1 hour after being separated

After the firefighters arrived at the scene, they were unable to put out the fire. The fire was raised to level 4 at noon. Zhang Yaosheng, who died on the first day of duty, entered the fire with his partner several times from dusk to evening, and was separated. He was rescued from the fire more than an hour later by his colleagues , but Zhang was unconscious at the time and died after being sent to the hospital for rescue.

Insufficient gas bottle stock after Xu's entry

As for Xu Zhijie, who died on the third day of the fire, he entered the fire with three team members that day. His captain found that Xu's gas bottle was insufficient. After handing over work with another team, Xu was gone when he turned around.

Xu was carried away from the fire about 20 minutes later and died after being sent to the hospital for rescue.

The testimony in court stated that between Xu's disappearance and his recovery, the "Mayday" procedure was also not initiated.

Fire Chief Chen Yili, who inspected the equipment and reminded Xu of the remaining working hours that day, agreed that if "Mayday" had been activated at that time, it would be helpful to coordinate search and rescue operations.

However, Chen revealed that since joining the Fire Services Department to the day of the incident, he had never participated in a Level 4 fire rescue, had also served as an entrance commander, and had never experienced the "mayday" procedure.
