In 2016, a four-level fire broke out in the Shichang mini warehouse in Amoy Industrial Village, Niutoujiao. Two firefighters were killed fighting the fire. The coroner's court launched an inquest into the incident last September.
Among them, Xu Zhijie, the fire brigade commander who died on the third day of the fire, entered the fire scene in the evening, and his supervisor found that Xu’s oxygen bottle was not under pressure. He tried to notify the entrance command station twice, but received no response. After that, the two were in the fire scene. Lost.
Later, when firefighters found Xu's figure, Xu was pulling the metal partitions in a storage bin. Others slapped him for about half a minute, but Xu didn't respond. It took 8 minutes to carry Xu out. After being sent to the hospital, it was confirmed that Xu was dead at the age of 37.
Another testimonial stated that the emergency search and rescue procedure commonly known as "Mayday" has not been successfully launched since he was missing until he was found.
The Coroner's Court ruled today (10th) that Xu and Zhang Yaosheng, another firefighter who died in the line of duty, both died unfortunately.
Xu Zhijie, the fire brigade commander who died on the third day of the fire (June 23, 2016), the second deceased in the mini-storage fire, joined the Fire Department in 1998 and was promoted to the fire brigade commander in June 2016.
Xu and his wife married in 2010 and have a son, now 12 years old.
His wife said Xu was in good health, had no serious or long-term illness, and had not undergone any surgery.

At 6:42 in the evening of the same day, Xu accompanied the senior defense captain Qiu Xiaosheng and other three people into the fire scene to carry out the warehouse explosion and fire extinguishing work.
At first, all the team members said they were in good condition. Later, the temperature of the fire site became higher and higher, and the fire hose suddenly lost water pressure. Qiu Sui instructed the team members to step back and squat to rest.
At this time, Qiu found that Xu's gas bottle pressure had dropped to less than half of the initial pressure, so he twice asked the command station to send someone to replace him by radio, but he got no response.
The replacement found no trace of the 3 team members
Soon after, the team members of the Qingshan Bay lift suddenly appeared. After Qiu realized that they were here to pick up the shift, he called the team members to retreat. At this time, the three team members had disappeared.
Qiu continued to use the radio to call the team members and search forward. Later, two of the team members, Xu Jianwen and Yuan Yaoqiang, appeared, but Xu was still missing.
At the beginning, Qiu insisted on staying at the fire scene to find Xu, but gradually became confused, so he could only climb to the exit along the light, and announced the news of Xu's disappearance.
About 1 minute later, Xu Jianwen and Yuan Yaoqiang escaped one after another. One of them lost his helmet and the other fell to the ground.

Two of them evacuated the fire by themselves
Xu Jianwen said that he had difficulty breathing when he entered the fire, and when he was confused, he felt someone retreat and immediately followed.
During the retreat, Xu was once bound by wires and tripped over. Xu should have stepped forward to help him up, and he has no memory of what happened after that.
Yuan Yaoqiang said that at the end of the fire, he heard a team member expressing the need to retreat, so he left with him. At that time, he never heard anyone say: "Kwun Tong pump retreat." He did not ask Qiu for instructions before leaving, so Qiu may not have retreated together.

As for Wen Zhiping, the then fire chief of the Qingshan Bay team who took over with Xu's first team, he said that when he saw Qiu Xiaosheng in the fire scene, there were no other team members beside Qiu.
Later, Wen found Xu and other two people in a storage warehouse at the end of the main corridor. At that time, Xu was constantly pulling the metal partitions in the warehouse. He was beaten for about half a minute without responding, and then collapsed against the wall.
Wen immediately ran out of the fire to ask for help, while another team member asked the rear team to come forward to support him on the walkie-talkie. During this period, the "mayday" emergency rescue procedure was not activated or it was for first aid.

Xu had no pulse when he was rescued
Wen then turned back to the fire scene, and then worked with the four team members to move the deceased out of the fire scene. Xu was finally carried into the fire scene at about 7:22. Chen Yili, the fire chief who had never activated Mayday during the process, checked equipment for Xu and others, kept in touch, and reminded the remaining working hours. He agreed that if "Mayday" had been activated at that time, it would be helpful to coordinate search and rescue operations.
However, Chen revealed that since joining the Fire Services Department to the day of the incident, he had never participated in a Level 4 fire rescue, had never been an entrance commander, and had never experienced the "mayday" procedure.
