The Hospital Authority today (27th) published the semi-annual issue of "Drug Safety", which mentioned two medical incidents related to drugs.

One of the cases involved a patient undergoing cataract surgery on the left eye. The doctor prescribed four types of eye drops for the left eye to be used before the operation. However, two eye drops were mistakenly instilled in the right eye during the first use.

The surgeon also evaluates the patient and decides to proceed as planned.

Interns give twice as much drip to diabetics

The latest issue of "Drug Safety" mentioned two medical incidents related to drugs. The first case involved a patient with a history of diabetes and high blood pressure who was admitted to hospital due to fever and was scheduled for surgery.

The doctor prescribed Dextrose-Potassium-Insulin infusion for the patient, and the infusion rate was 50 ml per hour.

However, an intern on duty mistakenly marked the infusion rate as 100 ml per hour in the "Inpatient Drug Prescription System", and two nurses arranged infusions for patients according to the system.

Another nurse noticed the infusion rate was wrong before the patient underwent surgery.

Another case involved a patient undergoing left eye cataract surgery. The doctor prescribed four types of eye drops, including Proxymetacaine 0.5%, Cyclopentolate 1%, Phenylephrine 2.5% and Tobramycin 0.3%.

However, when the patient was instilled with eye drops for the first time, it was mistakenly dropped in the right eye.

The patient and his son were informed, and the surgeon assessed the patient and decided to proceed as planned.

The HA reminds medical staff to carefully check information such as the drug, dosage, patient, method and time of administration, and pay attention to the location of the drug and the speed of infusion when prescribing and administering drugs.

In addition, the Kowloon East Hospital Cluster also shared in the journal the points to be aware of when prescribing anticoagulants to patients with dysphagia, including that some drugs are not suitable for grinding.
