A student at a school in Holon apparently accidentally received two vaccines of the same type, instead of a different type of vaccine - this is what the Ministry of Health said today (Wednesday).
The incident took place last week at the Kugel school, during routine vaccinations such as diphtheria, tetanus, pertussis and a vaccine against the papilloma virus, at the end of which a discrepancy was found between the number of students vaccinated and the number of vaccines actually given.
The PMI company, which is responsible for the vaccinations, on Monday sent an unusual event report by the company to the Ministry of Health, according to which one of the children apparently received two vaccines of the same type instead of the two different vaccines. The Ministry of Health is investigating the incident and its circumstances, but from the findings of the initial examination it appears Because it is a human error.
The Ministry of Health held a conversation with the company's management yesterday evening and began an in-depth investigation.
This morning the ministry instructed all the vaccinators in the schools that until the end of the investigation the vaccinations must be given at different times and not at the same time.
During the day, a conversation was held between the district health bureau and the principal of the school, and talks are also being held with the parents of the boys and girls who may be involved, according to the initial investigation, in the incident.
