Since the 19th century, French liberal doctors have been paid on a fee-for-service basis, that is to say that the patient pays his practitioner directly for each consultation.
All defended this mode of remuneration against the inclinations, in particular of the mutualist movement, to introduce subscription formulas.
The 1927 charter also made fee-for-service a fundamental principle of the profession.

But for several years, the public authorities have in parallel developed a fixed remuneration, with which the doctors receive a lump sum if they have respected a whole list of indicators.
Thus the remuneration on public health objective (Rosp), a kind of annual bonus granted to promote good practices, amounted to 5057 euros in 2021 for a general practitioner, 1391 euros for a gastroenterologist, or 1020 euros for a pediatrician.
