A 78-year-old patient died during a heart catheterization that took place this morning (Wednesday) at Beilinson Hospital in Petah Tikva.
He died as a result of a disruption in the catheterization operation, despite the resuscitation attempts made by the doctors after they noticed the malfunction.
The source of the disruption caused in the catheterization process originates from a malfunction in the x-ray system that allows the reflection of the blood vessels for the catheters, which stopped working in the middle of the procedure.
The hospital stated that the medical staff informed the patient's family and is accompanying them.
It was also reported that "the medical team itself is upset and shocked by the difficult and rare event, and is trying to digest and recover from it."
Given the circumstances of the difficult case, in coordination with the Ministry of Health, it was decided to establish an inspection committee, and at the same time representatives of the manufacturer of the device in Israel are inspecting it.
