A train decoupling accident occurred on the MTR Tseung Kwan O Line on December 5 last year. The government restricted MTR to submit an investigation report no later than tomorrow (4th).

MTR released an investigation report today, indicating that the investigation revealed that a ring used to fix the buffer in a semi-permanent coupler device in the sixth car of the train was loose, causing the buffer to shift in the internal structure of the coupler. The aisle between the cars appears to be stretching abnormally.

According to the investigation report, the workshop passage between the two trucks was still connected at the time of the accident, and the train did not deviate from the track.

The committee believes that the most likely cause of the loosening of the collar is that the original factory did not effectively use the screw fixing agent (Loctite) on the thread, so that the collar could not be permanently fixed, and after many years of use, the collar became loose.

As for the coupler device, the MTR has established a maintenance system for regular preventive inspections and regular overhauls. However, because the buffer is a proprietary design, the buffer manufacturer conducts a major overhaul about every nine years. No anomalies were found in the accident buffer when it was last overhauled in 2018.

The committee believed that the company had arranged for the original factory overhaul of the buffer according to the specific requirements of the coupler device supplier.
