Home hospitalization (HAD) concerns patients with serious, acute or chronic pathologies. If the clinical situation of your loved one justifies it and if his housing conditions allow it, he is likely to be offered.
It can be set up on the prescription of a hospital doctor or the attending physician, with the consent of the patient and his entourage. Admission is decided by the coordinating doctor of the HAD service on the basis of a therapeutic project. Care protocols are then planned by the care coordination team.
Housing conditions
The concept of domicile is very broad, since it covers personal housing but also collective accommodation establishments (such as retirement homes). Before implementing a HAD, the hospital's coordination team evaluates the patient's home to ensure that the patient's environment is suitable.
