The suspected knife attacker, who is said to have stabbed two elementary school children in Duisburg, is said to have previously announced a murder in a chat.
A witness reported this to the police in Bavaria in January.
This emerges from a report from the Ministry of Justice to the legal committee of the state parliament.
Duisburg - Measures had already been considered and taken against the 21-year-old, but had not been implemented before the crime on February 28th.
It is being investigated whether there were delays due to an incorrectly entered file number and documents not being provided.
A supervisory review has been initiated.
