According to the police, an officer was injured as part of Hansa Rostock's promotion to the 2nd Bundesliga.
During the game, fans fired massive pyrotechnics, the police said.
Occasionally, fireworks were also thrown in the direction of the emergency services, and an officer was injured in the eye.
Hansa Rostock had returned to the second division after nine years with the 1-1 draw against VfB Lübeck on the last day of the third division season.
The Rostockers were allowed to play the game on the basis of strict hygiene rules in front of 7500 spectators.
However, many fans in front of and in the stadium did not wear mouth and nose protection, among other things.
According to the police, there was a fan march in the run-up to the game, in which 2,000 to 3,000 people took part.
According to the police, "many pyrotechnics were ignited".
Lord Mayor expected rule violations
"I'm very excited about the third half," Rostock's Lord Mayor Claus Ruhe Madsen told NDR after the final whistle.
He hopes that many will behave sensibly in the face of Corona.
"We have to expect something to come out that we need to talk about," he said.
"But today we should celebrate."
Madsen had campaigned for the game to take place in front of fans.
He hoped to be able to prevent possible wild promotion celebrations of the supporters without compliance with the corona rules.
The state government in Schwerin had allowed fans in the stadium on Tuesday under strict hygiene rules, but numerous fans without protective masks could be seen on television.
The game was classified as a high security game.
