Sassuolo sacked their previous coach, Alessio Dionisi, on Sunday after the defeat against Empoli (3-2).
Since then, the club has been crushed at home by Napoli 6-1 on Wednesday in a late match and is in 18th place (out of 20).
It was his 16th loss of the season in the league.
Ballardini, 60, has coached numerous clubs in Italy, including Cagliari and Lazio, and most recently Cremonese in Serie B.
Sassuolo faces Verona on Sunday, a direct rival for maintenance.
