Excellent local health experiences as a starting point for real change in the Italian health service. This is the philosophy behind the book "The community that cares - From experience to the model of management of chronicity in the National Health System", produced with the contribution of Galapagos in editorial partnership with Egea, which tells what innovations are being implemented at the regional level to improve the care of chronic patients.

From the Family and Community Nurse as a new connector for territorial healthcare to the digital patient journey, these are the "success factors" present in the more than 40 best practices mapped in the volume, the result of meetings in eight regions: Piedmont, Lombardy, Emilia-Romagna, Tuscany, Lazio, Campania, Puglia, Sicily.A comparison that led to the selection of sixteen experiences that represent concrete models of response to the challenges posed by Ministerial Decree 77 on territorial assistance.

The research work that led to the drafting of the volume has identified eight main challenges for the health system, which affect the entire Italian territory and which are already being answered in the local realities examined, challenges ranging from the implementation of pathology networks to the digital transition in regional health systems to measure and facilitate the management of personalized pathways, improving clinical outcomes.

"Our health system is faced with the challenge of guaranteeing care to an increasingly long-lived population with a growing incidence of chronic diseases - underlines the Minister of Health, Orazio Schillaci, in a message of greeting sent during the presentation of the volume in Rome - to face this challenge effectively, it is essential to put the person at the center, enhancing local and home care, taking advantage of the opportunities offered by digital health and telemedicine and actively promoting scientific research. We are investing in this direction through PNRR funds to allow our National Health Service to make a qualitative leap, offering more personalized and targeted care to chronic patients."
