The Bremen-based residential and nursing home operator Convivo, which operates in several federal states, has filed for insolvency for key companies in the company.
Convivo announced this on Tuesday.
According to Convivo, it brings together more than 100 care facilities, most of which are located in north-west Germany, and employs around 4,800 people.
Several media had previously reported on the bankruptcy.
Convivo submitted the applications to the Bremen District Court on Monday.
The district court of Bremen confirmed on Tuesday that five Convivo companies have filed for bankruptcy.
The court had appointed a provisional insolvency administrator for each company.
"The group of companies is aware of its responsibility towards people and will accompany the next steps conscientiously," said Convivo's statement.
Measures have been introduced to ensure the care of the people cared for.
The wages and salaries of the employees are secured through the insolvency money until March.
It is not known exactly how many people were last cared for by Convivo.
The website says they are taking care of more than 18,000 people.
Convivo cited low occupancy rates in the area of inpatient care as one reason for filing for insolvency.
The use of temporary workers caused high costs.
The federal government's care reform also had a negative impact on the company.
Convivo also named price increases as a reason - for example in the case of indexed leases.
