District - The mission is clearly formulated: If the corona situation in the region worsens dramatically, as much capacity as possible must be available in the clinics. An order with which Dr. Felix Groß was entrusted with the district office. "We have this focus," emphasizes the medical doctor for the Garmisch-Partenkirchen district. For the physician from the Riegsee district of Hagen, one thing is clear: Such an approach is only realistic if the general practitioners are in the boat. The establishment of specialty practices for the so-called untimes, i.e. those phases in which the practices are usually not occupied, is the first step in this direction.

U-turn: Priority practices become an issue - large companies want to preempt the government

Groß researched in advance of his efforts. "A week ago, focus practice was not a big topic," he says. The U-turn came in the past few days. Since such a project could soon be ordered by the government, he wanted to preempt such an order. He read about models from Passau or Fürth. There, medical facilities were set up in schools or gyms, which are looked after by local doctors. "Our idea was not to go exactly this way," explains Groß, thus speaking for the general practitioners in the region. "We see a different approach as the right one, we are advocates of local resources." The model: family doctors join together regionally and regulate the best possible care for patients, and also care for nursing homes. "We want to start on Easter."

All details have not yet been worked out. "We will continue to work on this after Easter," emphasizes Groß. Regionally, various medical practices form a network that can be reached via a central number. The doctors share this telephone service themselves. "In addition, a second doctor is always on standby in case the other one actually has to move out in a serious case."

Groß emphasizes, however, that this procedure only applies to off-hours, for example from Friday afternoon to Sunday evening or on public holidays. "Then people shouldn't go to the emergency rooms of the clinics."

The construct will also have an impact on normal practical work. "Every doctor is already offering fixed times for infection consultations," emphasizes Groß. "This system offers the greatest possible flexibility." If a practice is already overloaded with treatments at one time, it can refer patients with suspected Covid 19 infection to colleagues who are offering a special consultation hour for these patients on this very day - and thus ensure relaxation. Groß assures that each of the doctors can offer their normal spectrum during the week.

Health care provider Great pride: Concepts for specialist practices are largely in place

The concepts are already well advanced in all regions - Ammertal, Murnau / Uffing (Nordlandkreis to Eschenlohe), Garmisch-Grainau, Garmisch-Partenkirchen (focus on Partenkirchen), Upper Isar Valley and Loisachtal (Farchant / Oberau) (see below). So far, only the central telephone contact is missing in the Upper Isar Valley. "Until we have them, emergency patients can also contact their colleagues in Garmisch-Partenkirchen for as long as possible."

Healthcare provider Groß is proud of what has been achieved so far: "This is really a completely new organizational structure that is unique in this form in Bavaria, possibly in Germany." However, the health authority remains the overriding authority, which orders the quarantine process of infected people - and tracked. The increased cooperation with family doctors is intended to prevent a potentially dangerous course from being detected.
