You can find this "declaration of movement outside his department and more than 100 km from his home" by clicking here.

Supporting documents required

The ministry, which emphasizes that "the distance must be calculated as the crow flies" indicates that it is necessary, in addition to this duly completed document, to have a "proof of address less than one year old", mentioning the name and the first name, in case of control. This residence, he specifies, "may correspond to the main residence or to a usual residence (secondary residence, residence of a geographic bachelor, etc.)".

Telephone, electricity, gas or water bills are accepted, as well as rent receipts, tax or housing tax notices, or proof of housing insurance, as well as '' a CAF statement mentioning housing-related aid.

Other document to provide: proof of the reason for the trip (proof of a medical appointment, school registration, summons from the police or the courts, certificate from the employer, etc.). Seven cases allowing these journeys are listed on the certificate:

  • That of the professional journey, which must be justified by the employer.
  • The journey between home and school, made by the student or his companion.
  • Travel related to consultations or specialized care.
  • Journeys intended to lend a helping hand to a vulnerable person or for the care of children, more commonly called "compelling reasons"
  • Travel linked to summons from the police, gendarmerie.
  • Travel linked to a summons from the judicial authority.
  • Routes linked to missions of general interest.
